Ineffective Health Maintenance: Nanda Nursing Diagnosis and Nursing Care Plan

Ineffective health maintenance is defined as the state of a person wherein he or she is unable to identify, manage and ask help in maintaining his/her healthy well-being.

Health maintenance is a very crucial process to a person because he/she needs to maintain a healthy body with proper functioning at all times. It is a crucial process because it entails a lot of resources in maintaining one’s health.

There are a lot of factors which may lead to ineffective health maintenance. These factors are the lack of financial resources, lack of knowledge on correct and proper health practices, inability in meeting the basic needs and lack of adaptive behaviors on changes in the environment.

These are the possible causes which may impede the maintenance of one’s health. Lack of financial support or resources is one of the major factors in health maintenance. Because being healthy means eating the right kind and amount of food needed by the body, takin necessary food supplements and undergoing routine check-ups. These entail a lot of money wherein if a person doesn’t have such, these will not be met.

  • Impairment or perception and cognition by either lack of fine or motor movements

  • Patient shows poor understanding of maintaining one’s health
  • Observed inability to meet the basic needs of life including the needed medications
  • Contradicting cultural, religious and health benefits

1. asses the client’s thoughts, feelings, beliefs, values and reasons of not doing the recommended plan of care It will determine the exact cause/s of not doing or following the recommended interventions
2. assess the socioeconomic status of the client which may influence their compliance to medical treatment It enables the nurse to understand further the client’s way of living as well as their cultural practices
3. assess the client’s sensory and motor skills It will help the nurse to know the possible barriers to a certain treatment given.
4. assess the clients’ financial status and presence of support groups It will determine the client’s capacity to comply with the prescribed treatment regimen and determine the presence of support from the family.

1. determine the client’s daily health care regimen This will guide the nurse on the complete prescribed treatment regimen for the client. It will serve as a basis for everyday care and treatment.
2. identify support groups related to the illness or disease process of the client This will let the patient be more motivated in the compliance of his/her treatment
3. help the client choose a healthy lifestyle It will prevent unwanted complications that may arise during the treatment
4. educate the client about the treatment regimen that he/she is going to take or undergo. Include the benefits of compliance to the treatment and possible effects if not completed. It will help the client understand why he/she needs such treatment. It will gain his or full compliance in the treatment regimen.
Collaborative
1. refer client to social services for financial assistance if necessary It is one way of the client’s coping mechanism to seek help if needed and to let him/her be more motivated to comply with the treatment prescribed.
2. refer client to community agencies for an appropriate follow up care It will increase the client’s social support regarding his/her health restoration
3. refer to attending physician if the client experiences worsening of health condition after a treatment has been administered This is to prevent any complications and determine the correct treatment necessary for the patient’s condition.
